Ann V. Koziatek, 55, of LaPorte, Indiana, passed away Thursday, February 21, 2008 at 6:41 P.M. in LaPorte, IN.
A Celebration of Life Service will be held at 11:00 A.M., Monday, February 25, 2008 at Springville Free Methodist Church, LaPorte, IN. There will be a visitation from 3-7 P.M. Sunday, February 24, 2008 at Cutler Funeral Home and from 10-11 A.M. Saturday, February 25, 2008 at the church. Burial will be in Springville Cemetery, LaPorte, IN.
Ann was born December 13, 1952 in Michigan City, IN to the late Robert and Norma (Crowe) Heath.
Ann was married on August 7, 1999 to David Koziatek who survives.
Other survivors include 2 Sons: Jason Archer of LaPorte, IN., Scott Koziatek of LaPorte, IN; 1 Daughter: Melodee Taylor of LaPorte, IN; 4 Grandchildren; 1 Sister: Rose Heath of Michigan City, IN; 2 Brothers: Joseph Heath of Michigan City, IN. and Allen Crowe of Indianapolis, IN.
Ann was preceded in death by her Parents.
Joan enjoyed spending the weekends on the boat and she especially enjoyed the motorcycle trips.
