Samayah Faith Kobat, 5 weeks old of La Porte, IN, died Wednesday, April 18, 2012 at Riley Hospital for Children.
She was born Sunday, March 11, 2012 in South Bend, Indiana , Daughter of Madison Kobat and Shannon Winters.
Surviving are her mother, father, 3 siblings, grandparents, and great grandparents.
A Celebration of Life Memorial Service will be held at 11 AM Wednesday, April 25, 2012 at the First United Methodist Church in LaPorte with Rev. Evan Lash officiating.
Cremation has taken place and arrangements have been entrusted to Cutler Funeral Home and Cremation Service, LaPorte, Indiana.
